Preliminary support for MSVC

Attached is the Initial support for MSVC. Debug and release version. This
was done on 0_8 branch.

Right now, the output and naming conventions are not what is expected for
correct execution, as I didn't have time to reconcile with the expected
output from the cygwin build. Everything builds however, if Stefan applied
changes to the source where incompatibilities caused errors in the compiler.
You will need to add a global include directory for Python to MSVC
(Tools->Options->Project->VC++ Directories->Show directories for : Include
Files).

I'm going on vacation for the next two weeks, I will probably be able to
touch up on this. Feel free to tinker.
