I'm using a similar setup, but without a smartcard.

I don't think that this has ever happened here, but I don't have that many 
visible programs on the first virtual desktop.

Maybe you can set Special Window settings or Special Application settings for 
pinentry-qt to keep its window on top. Right-click on the title bar of 
pinentry, select More Actions... and then Configure Special Window Settings...
Then add the "Keep above other windows" property (under Arrangement & Access).
