This may have been fixed in 0.21.0 release candidates.

There were a lot of changes made to deal with PKCS11 and NSS handling of slots.
#1975, #1935, #1961 and #1970 and maybe others.

This could also have someting to do with the setting in opensc.conf dealing with
exclusive use of a card by one application.
