OpenDLM lock module update

Hi all,

Just an update ... I've been looking at the handling of LVBs within the
opendlm lock module, after oopsing when doing ogfs_stat() in response to
df command.  It looked like the LVBs for the resource groups were being
forgotten before they should have been, that is, right after the RGRP
locks were released from the glock cache (but before the filesystem
unheld the related LVBs).

I've updated the DESIGN-opendlm document to reflect my current thinking
about it, and have implemented most of it here.  However, I have not
checked in any changes, because now I'm experiencing oopses relating to
lists of bhs attached to glocks ... df seems to work after the locks are
released, though.
