SFS and NAT

I have been running SFS now though a NAT/IPtables box for over a year.
I haven't been able to find any movement, but a while ago there was
suppose to be some work done to allow this to work more cleanly. I
currently specify a gateway as the SFS server in sfs's configs and run
'env SFS_HOSTNAME=gateway.example.com sfssd' which works well from
outside, but I can not get any internal systems to connect via SFS.
Has anyone been able to get SFS to work from systems both inside and
outside of a NAT? This hasn't worked since the system is known by two
different hostnames depending on if it's inside or outside.
Any tips/thoughts are appreciated.
