John,

1st question - I don't know since I do not use that calculator.

2nd - It is a fee to make/process the loan usually charged by the 
lending institution.

3rd - Most likely the principal adjustment will also be an adjustment to 
the interest expense in an equal amount.  At least that is my guess 
without looking at the annual bank reports of principal balance, 
interest expense and impound account transactions (if any) and comparing 
those numbers to the Moneydance calculator.
