WARNING - XFIXES FLAG DAY IMMINENT

I've decided to go redo the Xfixes Region operations to precisely match 
the server internal functions; the mess with offsets was just too 
confusing for me in practice, and if it confuses me, it seems likely to 
confuse others as well.

So, FixesExt, Xfixes, xcompmgr and the X server all need changing; prepare 
for CVS commits in each of those areas.
