linux 2.6.12.3 nvidiafb working again

Linux kernel 2.6.12.3 nvidiafb worked well for me on a PNY Geforce 4 TI 4600 
and a PNY Geforce FX 5200.  Doublescan modes aren't working correctly, so 
you'll need to disable them, but otherwise everything worked as it should at 
PC monitor frequencies.  I'll test arcade monitor stuff when I get the time.
