I'm answering my own question in hopes that it helps
others with similar problems.  It turns out that I had
too much system memory!  There is a problem with a
call to "dpmi_physical_address_mapping()" if memory is
above 512M (at least on my machine).  My guess is that
a virtual memory address becomes negative (when
signed).  My temporary solution is to stick with 512M
of memory.

Now I have a new problem.  I have configured advmenu
to  run fine on my arcade machine out of a Windows 98
DOS box.  I can also run advmame alone without advmenu
and it also runs games just fine on the arcade
machine.  When I try to run an advmame game from
within advmenu, the game starts to launch and then
immediately returns to the menu.  Again, this all
works fine from a pure DOS box.  Unfortunately, as far
as I know, advmame does not have a -logsync option
which really helped me debug the "too much memory"
problem that I had before.  Any help would be greatly
appreciated.
