One difference I notice when comparing a "good" trace against the "bad"
one is that in the good case, the CREATE_THREAD_DEBUG_EVENT events
all occur during the startup phase, i.e., during create_inferior.

I don't know if this makes any difference at all though.  What I find
mysterious is that the process-exit event never occurs.
