#999 should document column-major limitation of GDB

The DWARF information contains sufficient
details for GDB to properly handle row-major
vs column-major ordering of arrays.

But according to Joel, too much trouble to
fix, so instead we will document it as a
known limitation of GDB, with an appropriate
work around.
