Gramps uses two components to find the media files on the hard drive. In Preferences (menu >> Edit >> Preferences) on the General tab is the *Base path for relative media paths;* This points to the folder where you keep all your media files. i.e. c:\users\Public\Genealogy\Media (this is mine). https://gramps-project.org/wiki/index.php/Gramps_5.1_Wiki_Manual_-_Settings#General Then in Gramps each Media record has a Path filed. This has the final information to find the file in the base path.
