The format seems simple enough and usually elevation data is not complicated.
I would suggest that you write a small script that does the conversion for you.
Try to export the .grd file into something simple where you have the
format specification for and then write a small script reading this
and writing out the other stuff in the language of your choice.
(I usually use python for such tasks.)
