Yes, there were in fact two old bugs here, affecting both GMT 4 & 5:

1. The clip path for the entire region was hardwired to be a circle when -Je/E was used, regardless of -R.  In effect, it assumed -Rg.  This caused the blue circle.
2. The search that determines the min/max w/e/s/n for a region specified by -R...r did not check if either the S or N pole happened to be the antipode of the -Je/E pole [your case].  That caused a bad w/e/s/n setting and that gridline did not happen.

Updated in both GMT 4 (r10233) and GMT 5 (r13201); please update and check if your other issues further down (psxy/grdimage) still need attention.  If so please file a bug report on the tracker.
