How do I set the default page size

Some weeks/months ago an evolution install seems to have changed the
default page size from US Letter to A4 in the gnome print dialog.

Unless I manually change the page size each time I print, printing fails
silently.  My travails are documented in the Evolution bugzilla

This is the same whether I'm running under Gnome or KDE.

How does one change the default page size for gnome printing?  I've
tried googling and even trying to read some source without success.

For a bonus question, is it possible to change the default print
command, I'd rather use kprinter than lpr in order to be able to
use/control some of the advanced capabilities of my printer.
