I too was frustrated by this for the longest time until I read a suggestion (either in bugzilla or another Ximian list) to blow away everything in /var/cache/redcarpet and then re-run red-carpet. Red-carpet rebuilds that directory if it finds it missing. That worked for me. I read a later post that suggested that all you have to get rid of is /var/cache/redcarpet/icons. Give that a try.
