This is extremely strange. The patch indeed works with FreeBSD's patch, or with Emacs' `diff-mode'.

But indeed it is rejected by GNU patch, I can reproduce your exact output here. Even regenerating the patch with CVS doesn't work, nor lightly editing it.

Could you please try to apply it with another tool? You can use Emacs, or even git ('git apply universal_ctags-v2.patch' works). If you can't, please wait while I investigate (next week).
