Sometimes such things happen when the .tex file hasn't been
run through pdflatex "often enough". GNUmed does 3 runs by
default, recently I read about 4 having been necessary in
some cases, and I do recall reports of a sometime requirement
of 5 runs.

Should you be able to confirm that your problem consistently
goes away after 5 runs I'll gladly bump the GNUmed runs to 5.
