I would be highly interested in when and how this does happen,
too ! It certainly shouldn't happen "when creating an allergy
without a user entered narrative" ? OTOH, an episode without
any attached item is technically possible and hard to protect
against (should we, actually ?) since there are no "reverse
foreign keys" nor are there database "asserts".
