Looks great. I have two comments.

_IOAPPEND seem to only be in MS headers for POSIX compatibility. You may
leave that out if you don't want to have this in the mingw runtime.

Is "# ifndef ____MINGW_IMPORT" intentionally not
"# ifndef __MINGW_IMPORT"?
