[PATCH] RFC 1345 Digraph Input

Don't know if this will be useful to anyone, but I wanted slightly more
digraph support in screen than just the Latin-1 characters.  I added in
all of the digraphs in RFC 1345 (which is not all of the RFC; there are
at least trigraphs in there as well).  This necessitated removing some of
the defined digraphs because they conflicted; I removed all of the non-RFC
ones in the interests of simplicity.  I also had to remove the code that
took the constituent characters in any order, since the RFC defines
distinct digraphs that differ only in the ordering of the components.
