Text instructions can be included in the new issue form, they appear
directly in the text entry box for the bug, so they're pretty hard to
miss.

We already had some text in that form, however it lacked a link to the
report-bugs page, and was missing some details from our old Launchpad
bug form.  I added that link last night, and added some of the missing
text, although I worry about it being too verbose.

Take a look, edits welcomed; myself or any of the other gitlab admins
should be able to update it.
