Here's what I'm trying to shoot for:

I'd like it to intuitively know that 1st and 2nd level go  
horizontally, 3rd and 4th level go vertically in a template using  
outline mode. Is that an impossible feat to have happen without moving  
things around myself?

You can change the appearance of many aspects of a diagram by using  
the Diagram Style option (Format > Diagram Style....).  You can create  
your own diagram styles using File > New Resource > New Diagram  
Style.  Does that help?
