Here is a .zip of the result of using "File | Collect for Output," including fonts, on a test case demonstrating my problem.

On page 15 we see a lone "CHAPTER 12" chapter heading at the bottom of the first column, even though the paragraph style specifies "Keep with next paragraph."  I expect that this heading would be at the top of the second column.  On page 49 we see an example of this feature working properly, where the "CHAPTER 35" chapter heading appears at the top of the first column, but if we remove the paragraph style that heading jumps to the bottom of the last column on the previous page.

I'm running 1.5.7.svn on Linux.
