You don't say whether the alarm shows up on the screen but doesn't sound or just doesn't work at all. If sound, the problem may be in the headphone socket. If you use earphones when listening to PTunes and then retract the earphones, the socket may not revert to 'normal' speaker operation. Next time an alarm doesn't sound, try listening for it in earphones. If the socket is the problem, try squirting a little electronic switch cleaner in it and then insert/remove the earphone jack several times.
