I use Dokuwiki (http://www.dokuwiki.org/dokuwiki) for documentation as well as a web page. There is a version that runs from a USB drive. I don't think there is a version for phones, but since it is open source, maybe no one has asked. You can see one of my installations here, http://theportmans.name/wiki/.
