Survey Tools (pop-up type)

As part of a diversified effort to better understand our users (we're doing
several other types of testing as well) we would like to implement a short
pop-up survey on a client site.  We would prefer that this not be something
that would take the user away from the page, but rather something that would
appear on the page as a pop-up (we don't care what technology it uses,
wheher it be a hidden DHTML layer, jscript, or something else).  It is a
very short survey, ~7 questions.

Ease of use and implementation are our primary decision factors, not
necessarily cost (i.e. if it's not a free tool that's OK with us).

This will directly inform our (re)design process as well as our strategy.

I thought perhaps there may be some on this list who use such tools.  Those
who have first hand experience are preferred. Posting to the list would be
fine, or respond to me directly via email.
