Summary: Dialog "Run/Debug" needs polishing (at least on Mac)

Initial Comment:
Below is a list of the issues

All those do not prevent the run/debug to work, but
they are potentially misleading for users.

1) Screenshot #1: the text for the two fields is cut on
the left. (However, it appears correctly and I can edit
it, once I've clicked on it). The "Arguments" tab has
the same issues: the whole panel is cut on the left side.
2) Screenshot #2: the tab for the runtime to use has no
header
3) The location of the PHP exutable has to be setup twice: 
- once in the Preferences
- once in each runtime configuration. 
Instead, the runtime configuration should take its
defaults from the Preference
4) There are two tabs called "environment". One of the
should "PHP configuration"
