i use amavisd-new , and its integrated spamassassin/virus checks.
There you can configure mysql lookups (i think also ldap..)
I was able to integrate the amavisd-new mysql lookups into a
mysql spamassassin module for squirrelmail webmail frontend.
(the structure had slightly different formats..)

there users can login and set 3 values: tag-level , some higher level, and
kill-level
with those values defined in the mysql the amavisd-new can tag the mail in
2 ways and can drop it also.
with those tags, you can use procmail or sieve to filter the mails into 2
spam level
boxes.

well my amavisd-new is like 1 year old i know there are even more features
by now.
