There's examples online... For example: http://www.azxws.com/ispman/ispman-handbook.html Most people are moving to Postfix 2.0 though. For load balancing, most people use round robin DNS. This isn't perfect load balancing, but typically close enough. A nicer alternative is Linux Virtual Services (http://www.linuxvirtualserver.org) which is much more intelligent, and can handle high-availability and load balancing for IMAP, POP, HTTP, etc as well.
