I think I found it.  According to the schema in /etc/openldap/schema,
the objectclass 'top' MUST have a dc attribute.  Adding a dc attribute causes an
error.... Because top doesn't define the dc attribute. So I had to add the
objectclass of dcObject.

Adding those two lines to the tmp/ldif/ispman.ldif made make ldif-install work.
Now to see if I can get the old data to go into the new tree.
