Newbie Setup problem

After putting the new .conf files in place, When trying to run ldapadd
-f tmp/ldif/ispman.ldif it returns with:

SASL/OPT authentication started
ldap_sasl_interactive_bind_s: Internal (implementation specific) error (80)
         additional info: SASL(-13): user not found: no OTP secret in database


Can anyone get me heading in the right direction to correct this, I
have no idea where to start or even what the problem actually is.
