You did not answer my question: How did you (as nurse) fill teh first 
survey? With mostly "yes" answers or mostly "no" answers? Only a survey 
with mostly "yes" answers will genrate a job for the supernurse..

When you say "i tried login as admin and create a wellness, the supernurse 
still cannot evaluate the survey." I feel maybe you have not read what the 
process is supposed to be..

this done.

but i cannot see the supernurse worklist after the nurse approve it.

yah right, health risk evaluator is supernurse

the nurse becoem a survey filler.
