Oh, sheesh.  I figured it out.

Reading the source code helps.  :)

The problem is, I don't have my proxy servlet set at the root of the web 
server, but inside of an application, thus my Prefix parameter needs to 
include the entire prefix, web application included...

<param-name>Prefix</param-name><param-value>/lvs4/google</param-value>

Duh.
