Sounds like a non-recent version of jetty. I think there was some
situation where the endpoint of a request was being reset before
the log message got written, hence the NULL for the remote address.
I can't find the bug that was related to it, but I checked the
code for 7.2.0 and it shouldn't happen with that release.
