Just got through having a dialog on this mail list
about that - see below. I just used the URL that Steve
Davidson sent (it's in this message). Then Sash
outlined the 3 possible solutions.

As it happens, I debugged this issue this weekend, and
fixed in CVS.
If you look in the system.xconf for app-navigate, the
createdb model is
defined for the "Keel" menu, but "Keel" is allowed
only for users in the
"root" group, which means that you'll only see the
Keel menu after logging in,
which will only happen after the tables are created. 
Nice chicken-'n-egg
situation!  (The part about protecting the Keel menu
for "root" users happened
  between the writing of the tutorial and Keel 2.0
release some time).

So, solutions:

- Access URL directly as suggested by Steve
- Take out "<groups><group>root</group></groups>" from
the Keel menu config
- Add a new top-level menu item that allows
unrestricted access to createdb
- For Keel-2.1-dev I chose to move the createdb model
under the "Data" menu
