Summary: CounterMonitor behaves strangely on rollover

Initial Comment:
The specification is not very clear what happens to the
threshold when a modulus is defined but the observed
value is incremented greater than the modulus value.
Behaviour of the RI also varies depending on whether
the counter itself rolls over.
