Folks,

Attached is an updated NetBeans JCA, utilizing PDF forms.  It allows someone to electronically enter all fields via Acrobat Reader, except for the Signature and Date.

As someone who recently filled out the JCA, I can attest that this is more far more convenient -- and readable -- than writing one's information in.  Let me know if this is something you folks are interested in, and if so, if there are any other changes you folks would like.
