refAllLinks() fixed, TransactionEvent added

This message is for the MDR developers to avoid double work:

I've made the following changes to MDR:

- RefAssociation.refAllLinks() now returns an immutable live collection,
  it returned an immutable dead collection before
- class TransactionEvent added, events fired on repository at start, commit
  add rollback of an outermost write transaction

I'll provide a diff the next week when Martin is there for review.
