Ok, the build finally was "Successful", using the steps below.  Note the 
-P on the cvs checkout command.  Running what you suggested without the 
-P gave me a "Build Failed" with an error about an existing javahelp 
directory, and it told me that I needed to use -P to avoid checking out 
empty directories. The exact error is long lost in the scrollback on my 
shell, though.  I'm guessing this could be related to the fact that it 
was partially built, so I deleted everything, and tried again from the 
start, with the -P option.

So, as I said, the builds completed "successfully", but I'm not 
convinced that MDR is installed properly.  I understand now that it is 
supposed to be included on netbeans 4.0+, but after looking at the 
instructions for using MDR, it seems that access is entirely through the 
View menu.  Neither installation (Netbeans 4.1 or the build that just 
completed) has anything related to MDR on the View menu.

The View menu options common to both installations are: Editors, Code 
Folds, Web Browser, Documentation Indices, Show Line Numbers, Show 
Editor Toolbars.  The Nebeans 4.1 installation also has a "Class Details 
View" option.  The instructions for using MDR also refer to 
Tools/Options/MDR, which is not there on either one of these installations.

What am I missing?
