I just added some "example" labels to the New Query dialog so it is a
bit clearer as to what goes in the fields for now.  I also fixed the
exception that gets thrown if they don't enter anything and hit OK.
Hopefully it is a little better now.  :)

I was wondering how I get this code pushed back into the 3.4.1 baseline.
Cause I would really like to get this module out to everyone here at
work, but they are all running 3.4/3.4.1, not the dev version.  So let
me know if it is possible or not.
