File a bug please. Probably part of a larger problem that UI decided to 
remove icons from *most* actions for purposes of the menu bar, but now 
toolbar buttons suffer.

You don't need Attach to debug NB code. Right-click a project you are 
patching and select Debug. Or set it as the main project and press Debug 
Main Project.

As elsewhere, there are special circumstances where you want to start 
the app differently; I'm just saying that these don't necessarily merit 
a toolbar button more than other actions people run.
