Potential concurrency problems with osworkflow v2.1.0

We are using osworkflow 2.1.0 (and oscore 2.0.1) in a multi-threaded application.
 
We are seeing some very strange behavior where getAvailableActions in AbstractWorkflow is returning nothing when there are in fact defined actions.  If we stop and restart our application it (usually) starts working again.
 
Wondering if anyone has seen this and how heavily osworkflow is really being used in multi-threaded applications?
