[Opensymphony-oscache] Pages only caching some of the time

This is my first time posting here.  What would constitute a test case so that you could see it?

Here is the form on the live site.

On the left sidebar you will see that the last radio button is selected: "View all on one page".  Then simply clicking "update" just below that will re-submit the page.

OSCache hasn't been implemented on the live site, we are first integrating it on the dev site first, which for all purposes is the same as the live one, but not publically accessible.

To recap the problem with this page in mind, if I sit there and click the "update" button about 50% of the time it creates a new cache file, and the other times it reads the cache correctly.  Since I'm not changing any options before I click "update", the URL is remaining the same, and the data/content behind it, although read from a database, is not being updated or changed.
