Summary: some optimistic expressions have no host

Initial Comment:
Some optimistic expressions appear to have no host,
which is very bad since it means an error will occur if
they ever need to be unoptimised.

I have reached this conclusion by putting a host==null
check at the beginning of
FixedAppExp_0.getValue(Interpreter). This gets called
tens of thousands of times during the heap build.
