[OFF-TOPIC?] - SOAP Fault should be defined? Where?

Greetings,

I'm working with SOAP, and till now I already defined a schema that
should be applied (verified) in all the SOAP content (body) of my SOAP
messages. Now, since I'm not experienced with SOAP architecture, I
intend to clarify a doubt here if possible.

- When, for any reason, I must throw a SOAP fault I must define the
faultcode (processing code), faultstring (human readable), faultactor
(URI of who caused) and a detail part (could be the request soap
message's body content - if not understood, or a custom (optional)
content if needed).

My question is, where these definitions should be specified? Should my
original schema be aware of this possible errors? Is there any specific 
schema for this issue? Is the SOAP schema aware of this? It makes sense
that exist any schema for this issue? The (fault) actor is abstract here
or must be also defined (validated) here?
