Fix account visibility bypass and PII leak in QueryAccounts

This follows up on commit 69e18414050b9cbda587d8cfe1475a4747376d7b.

Enforce AccountVisibility policies in QueryAccounts and restrict
detailed PII expansion (o=DETAILS and ?suggest) for unauthenticated
callers when accounts.visibility is restricted (SAME_GROUP,
VISIBLE_GROUP, or NONE).
