(I'm CCing the KPkgManager author).
I have looked at the kpkgmanager source. I couldn't exactly grasp how is it 
designed, but from readme i deduced this:
1) there is a kded module doing the "dirty work" - i particularly don't like 
this, as it introduces a whole slew of issues... we can probably discuss the 
details later
2) It seems that it uses kio-apt as a backend. It is a design decision i don't 
agree with completely. I get the feeling that it should use kio-apt for 
installation as well, which has some issues as well (most of which are shared 
with point 1).
Another thing i thought of was that package management system independence. I 
can't  imagine how would this be achieved, given the design differences among 
disperate systems. As an exercise, you could try to port it to libcapture, 
which is simply an libapt-pkg frontend, but has quite different 
characteristics. I have this idea that your  frontend (kpkgmanager) would 
have to be very complex to handle all this. But you know the best. Shedding 
some light on this would be certainly educative for me.

As for the "fix KDE/Debconf" item on your TODO: could you please elaborate? I 
am the KDE Debconf frontend author and i'll be glad to help fixing it if i've 
broken something :).

I'm also working on a different apt frontend for KDE. It is still unusable 
(the package manager (dpkg) interface is missing), but it is getting there. 
It is called kapture and is composed of aforementioned libcapture (which 
wraps apt-pkg in a frontend-friendly way), libkapture, which contains KDE 
widgets and such for dealing with information exported by libcapture and 
different frontends using that (currently one kpart and one KMDI-using). I 
planned on writing an io-slave as well based on the libcapture library, that 
would invoke a simple part based on libkapture for installation (or any other 
non-read-only operation). All of libcapture-based apps can run in both 
read-only (non-root) and read-write (root) mode. The distinction is still not 
implemented very well, but it is planned.
