My apologies, I had been testing with Dolphin. I took a screenshot of the wrong
window.

I am now able to reproduce this on Plasma 6.3.3 after resetting Dolphin to
defaults by renaming this directory (that contained only dolphinui.rc)

❯ mv ~/.local/share/kxmlgui5/dolphin{,.bak}

I'm attaching the original, and I'll let the Dolphin maintainers take it from
here.
