This bug is still around in KMail 1.9.7 (KDE: 3.5.7-0.1.fc7 Fedora).

I'm using SSL and clear text password (and dIMAP).

The "accept forever" seems to only apply to the current session, or maybe to the current IP address of the server. However, it seems to be forgotten when the server changes IP. Is it so that the "forever" setting only remembers on IP address for a give account? Where is the setting stored? Is it in some kind of table or in a datatype that can only take one value?
